Methodology
How Guardian scores clinics: weights, thresholds, and peer benchmark construction.
Composite score weights
How the headline 0–100 score is assembled
Tier thresholds
Minimum composite for each tier
How we credential a clinic
Four-stage review with a standing on-site audit cadence
Intake & eligibility screening
Accreditation body verification, regulatory standing, malpractice history, and baseline operational KPIs collected and validated against primary sources. Minimum: active JCI or equivalent national accreditation.
Document & data room review
Clinical outcomes for the last 36 months, credentialing files for every surgeon, infection control logs, and patient satisfaction raw data are scored against the JetPatient 86-point rubric.
On-site audit
Two-day physical audit by a JetPatient clinical director plus an independent reviewer. Walk-throughs of OR, ICU, sterilization, and recovery. Surgeon interviews. Anomalous findings trigger root-cause review before tiering.
Outcomes baselining & continuous monitoring
First 25 referrals are baselined against the peer cohort. Every subsequent episode is tracked through discharge and 90-day readmission. Composite score recomputed daily; tier reviewed monthly.
Peer benchmark construction
How we define “better” fairly across geographies
Risk adjustment
Each episode is risk-adjusted for patient age, ASA score, comorbidities, and procedure acuity before comparison.
US in-network cohort
Medicare limited data set plus commercial payer benchmarks drawn from licensed HCCI and FAIR Health feeds.
International median
Weighted median of credential-verified facilities in the JetPatient network, excluding the clinic being evaluated.

What employers see
Employer reinsurers and procurement teams ask for “as-of-today” credential status, not as-of-intake. Continuous Monitoring is a downloadable artifact in every Guardian quarterly review — the foundation of stop-loss negotiations.
